Office Policies and Mission

Office Hours

We are open to serve you, answer questions or schedule an appointment during the following hours:

Monday – Thursday: 7:30 a.m. – 5:00 p.m.
(Closed from 12:00 p.m. – 2:00 p.m. daily for lunch.)

Our Mission

We are committed to providing the highest level of quality dental care, utilizing the most advanced technology available. Our goal is to treat all of our patients with courtesy, respect, professionalism and gentleness. We strive to educate our patients to enable them to make informed choices on their dental care and oral health.

Appointments

We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day. Calling in advance is always recommended.

Your scheduled appointment time has been reserved specifically for you. We request 24-hours’ notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment; however, after missing your second appointment without notifying us 24-hours in advance, you are subject to being charged a broken appointment fee. Please call us at (970) 945-8753 with any questions or to schedule an appointment.

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Please inform us of any changes to your insurance coverage.

Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If claims are not paid within 60 days, you are responsible for the balance. If you have any questions, our courteous staff is always available to answer them.

Privacy Policy

This HIPAA form, Notice of Privacy Practices, presents the information that federal law requires us to give our patients regarding our privacy practices.

If you require any more information or have any questions about our privacy policy, please feel free to contact us. The privacy of our visitors is of extreme importance to us. This privacy policy document outlines the types of personal information that is received and collected by Dr. James C. Setterberg, D.D.S. and how it is used.

Personal Information

By entering your full name, e-mail address and phone number, you are providing personal information that will be used by our office for the sole purpose of returning your request to be contacted by us. We will only use this information to contact you in order to assist you in scheduling an appointment to be seen by Dr. Setterberg, and/or to answer any questions you may have indicated in the comments section. Our intention is to only use your personal information to return your request for contact regarding a dental appointment, and/or a dental related question.

Opt-Out Option

Please contact us if you wish to opt-out/unsubscribe from receiving any future communication.

Infection Control

Infection controls and universal precautions protect clients and staff alike. Everyone benefits from rigorous infection control — you, your dentist, and the dental team. The cornerstone in a good and safe dental practice is the element of trust. You should feel free to discuss this topic with Dr. Setterberg and receive a straightforward answer.

Dr. Setterberg and our entire team follow procedures recommended by several federal agencies: the Occupational Safety & Health Administration (OSHA), the Environmental Protection Agency (EPA) and the Centers for Disease Control (CDC). These measures include:

  • Disinfectant hand soap
  • Gloves and facemasks
  • Chemical disinfection of countertops and surfaces
  • Sterilization of all equipment before every use
  • Disposable materials

We sterilize all reusable equipment, including dental hand pieces. We use an autoclave, a device that kills bacteria and viruses by steam, heat and pressure. Our sterilization equipment is monitored weekly and monthly in accordance with OSHA compliance.

The best defense against disease is information. The more you know, the better equipped you are to make wise decisions about your health care. The more you know about our daily procedures and policies, the more comfortable you will feel.
